PDO: Team Sign Up Sheet

Note: if you don't have a team, you can sign up individually, and we'll try to get you hooked up. Don't fill out this form, but use the Individual Sign Up Form.

Red text denotes a required field. Full information for Captain and Co-Captain, and (at least) names for "Player 3" through "Player 4" are required.

Fill in the required fields, then click the "Continue to Additional Players Page" button at the bottom.

Pub Name is required. You can select a participating pub from the drop down list, or enter a new pub by typing in the name and address information. A complete list of participating pub addresses is available. If your pub is not in the drop down list, you must provide full address (including zip) and phone number for your pub.
